IBN Insurance Information and Membership Application

Introduction

You can now join IBN at the same time as you purchase your products liability insurance coverage!

Coverage is immediate. The details are below in the membership application. Simply complete the membership application here, choosing the member category for you. Then submit your application and you're covered! If you have questions about this change, email IBN and put "MEMBER CATEGORY QUESTION" as your subject.
If you have your insurance elsewhere or are applying for Apprentice membership, you do not have to purchase insurance in order to join IBN! Just check the membership option for "Full Indie Without Insurance" or "Apprentice Indie" in the application below! It's that easy!

You are probably here because you want to become a part of an organization with a proven track record of helping people just like you launch, own and maintain successful businesses. IBN is that organization! Use this form to join IBN. If you would like to first view our many membership benefits, click here. Membership requirements are simple: either you are a supplier of raw materials, bottles and other supplies, in which case you are a Supplier Member, or you are a manufacturer of products, in which case you are a Full Member. If you are both a supplier and a manufacturer, you would apply for Supplier Membership. If you have questions about completing this application, email us!!

All information on this form is required in order to process your IBN membership application. IBN does not sell, rent loan or give away any personal data. All form fields MUST be completed in order to fully process your application. Membership is $150 or $175 per year, depending on which membership type you choose -- see below for membership options. (Insurance is of course additional if you purchase it.)

The information you provide here is used to compile IBN's Member Directory and, if you are applying for a Full & Supplier membership, for your MangoButter.com Power Profile. Send updates to us or they will not be included in the Directory.

Choose A Membership Category. Our membership category options have changed due to the addition of our Products Liability Insurance Program! Please read carefully and choose one of the memberships below. If you already have insurance through another company, or if you are not a cosmetics manufacturer and you wish to join without insurance, you can do that. If you are a cosmetics manufacturer, and you have your insurance elsewhere, after you join, please fax your insurance certificate to us at (801) 763-1374.

Section II. Username and Password Selection

Usernames and passwords are not case sensitive.

Your IBN username will be the email address entered in this application. This sername will be used to access the password protected pages on IBN's site.

Choose A Password (minimum of 8 characters; use only lower case alphabets and numbers). Do NOT use "soap" or "soaps" as both are not surprisingly already in use. This will be the password you will use to access the password protected pages on IBN's site.

Please keep your Username and Password in a safe place. Sharing of your Password may result in discontinuance of your membership without refund. You will be notified by email of the uploading of your Username and Password, which should be completed within a total of business 24 hours.

Section IV. Applications For Supplier IBN Memberships (Skip To Membership Directory Listing If You Are Not A Supplier.)

If you are a supplier, please provide us with a 80-word description of the products and/or services you supply to other businesses. The information you provide will be used on IBN's Supplier Site listing, MangoButter.com. Please provide your description in the space below.

If you are a supplier, please provide us with several keywords and phrases for the MangoButter.com search engine. Separate your keywords with commas.

If you are a supplier, please select at least one category for your company to be listed under in the MangoButter.com directory:
To select multiple categories, hold down the Ctrl key while selecting.

Section III. Membership Directory Listing

If you are applying for Full or Supplier Membership and would like to be included in IBN's membership directory, provide a no more than 75-word description of your business for IBN's Membership Directory Page and our written directory. All members must be included in the directory so please provide a description of your company for use in our directory. Take a look at some of the descriptions currently at the Membership Directory for an idea of what is expected. Please conduct a thorough grammatical and spell check of your submission. If you go over 75 words, we will edit your submission down. If you do not complete this section, we will create a description for you. Please do not use all caps in your description.

I understand that if I have the products liability insurance member benefit, the policy will expire a year from today. I further acknowledge that the insurance broker will not send me a renewal notice and that it is my responsibility to renew my membership with insurance before it expires 12 months from today, or the policy and IBN membership will lapse. This does not apply to me if I am renewing without IBN's insurance program.
This section applies only to cosmetics companies: I understand that compliance with the Fair Labeling & Packaging Act and the US Food, Drug & Cosmetics Act is my responsibility, and that I can find information about both at the FDA's website at this link.
